Some institutions allow you to sit it for credit as early as third year, whereas other institutions won&#8217;t allow you to sit it for credit until your sixth year?

        <p>IMG in British residency with USMLEs. Thinking about US Fellowships after my residency in vascular (surgical not endovascular) and also minimally invasive spine.

Which are the good places that take IMGs for hands-on fellowships?&nbsp; Prefer busy clinical and operating to research and some supervision/training.

Which offer &#8216;Clinical Instructor&#8217; / Junior Faculty level?

        <p>So&#8230; apparently the the work hour gremlin thing is coming around the corner again&#8230; or I got used to its smell.

While it may be a good thing for many, it seems like it may ultimately be more detrimental to Neurosurgery (in my opinion). 
<br />
Feel free to enlighten if you think this is a wrong assement.

but I haven&#8217;t heard what else neurosurgery could do to its defense. Do we all need to be more vocal, proactive in some way?

Sure, more scut could be cut out, and more resources could be put into optimizing resident teaching/operating time/rest...but I don&#8217;t see who would want to pay for that.

Just trying to see what others thoughts/ideas/understanding/suggestions may be. If its going to happen perhaps some discussion, or preemptive action could be taken.....

        <p>I am just finishing up internship at one of the top institutions in the country.&nbsp; I did a month of Neuro Trauma, and absolutely loved it.&nbsp; It was crazy in terms of hours, but the whole watching surgeries on the brain and acting swiftly experience was awesome.&nbsp; I had never done a NS rotation before, otherwise I am sure I would have gone into neurosurg to begin with.&nbsp; I am having thoughts about switching to Neurosurg, but I am not sure how should I go about doing it.&nbsp; I am afraid to talk about it to anyone at my program.&nbsp; and what is the likelihood of landing a PGY-2 spot?&nbsp; should I just apply though match?&nbsp; I have no idea how I would get letters.&nbsp; any advice?

Specifically- would like to get both endoscopic training and open experience

Here is what I am aware of:

USF- Van Loveren- great guy, great fellowship- no endoscopic
<br />
Portland- Delashaw- great guy, potentially best fellowship in the county right now
<br />
Spetzler- lacks luster due to 4 chiefs/yr there- 
<br />
Pittsburg- Gardner- no one knows right now how good this is
<br />
MD anderson- new program- not much known
<br />
Hopkins- don&#8217;t know much
<br />
Utah- filled till 2013

I am open to all opportunites- please tell me what you know!!

        <p>Hey I&#8217;m curious - what&#8217;s it like...how are you treated as a newly matched resident?&nbsp; I mean everyone labors all year interviewiewing applicants etc., and you may be the one guy/gal that materializes for the program (if they&#8217;ve got one spot per year, for example), do they welcome you into the fold with open arms?&nbsp; Is it a nice experience? Anticlimactic?&nbsp; Fun?&nbsp; Impersonal?&nbsp; Exciting?

        <p>Once again, I apologize if I am infiltrating this forum as a pre-PA but I figure this one of the best places to get a perspective on the Surgeon-PA relationship in the OR.

So, how much does the Neurosurgical PA do in the OR, in a typical case what would they be doing perioperatively? I start a surgical PA program in the fall and really want to do NS and just want to know how much hands on time I will have in the OR.
